Well, 6 is usually enough:

   bash-2.01# grep level /usr/lib/mgetty/voice.conf =20
   # Possible log levels are:
   voice_log_level 6

Your file might be elsewhere. Use locate voice.conf or
strings `which vgetty` | grep voice.conf to find out.

Yes and no. It contains enough to see the problem, however I can't see the
voice.conf and settings with this debug level :(

Also, next time please:
   - try a current version (1.1.14 is very old, current is 1.1.21 +
     patches)
   - include your log file in the mail, do not attach, it makes
     usage more complicated.
   - only include a log file for one or two calls, do not simply
     paste your complete log file.

Now, my comments:
09/21 10:43:57 yS0 playing voice file /var/spool/tmp/messages/standard.rmd
09/21 10:43:57 yS0 vgetty: Could not open voice file
09/21 10:43:57 yS0 vgetty: Could not play greeting message
09/21 10:43:59 yS0 recording voice file /var/spool/tmp/incoming/va20332.rm=
d
09/21 10:43:59 yS0 vgetty: Could not open voice file
09/21 10:43:59 yS0 vgetty: Could not record a message

So, does the file /var/spool/tmp/messages/standard.rmd exist and is it
accessible to the voice user ? (named as phone_owner and phone_group
in voice.conf)

Does the directory path /var/spool/tmp/incoming/ exist, and does the voice
user have access to it ?

I hope you subscribed daimon@galaxycorp.com and not daimon@galaxycorp.co :)
