Dave Van Buren (and some others?) implemented in-db MI on JHM several years 
ago.  They posted a reference (along with the help text) on moo-cows a 
long time ago in response to a similar question, so you might either 
search an archives of the moo-cows mailing list for MI, or else look on JHM.
